The health information is kept confidential, for camp staff use only.

Parent or Legal Guardian is responsible for providing written instructions for their child’s medication. YOUR CHILD MUST BE ABLE TO ADMINISTER HIS/HER OWN MEDICATION. (MEDICATIONS MUST BE IN ORIGINAL CONTAINER).

It is understood that the Camp staff will take all reasonable health and safety precautions and is relieved of all responsibility in case of sickness or accident.

EMERGENCY MEDICAL AUTHORIZATION
To enable parents or guardians to authorize emergency treatment for children less than 18 years of age, who become ill or injured while camping. In the event reasonable attempts to contact parents or guardians have been unsuccessful, I hereby give my consent for my child (1) to receive emergency first aid or the administration of any treatment deemed necessary by a licensed physician or dentist, (2) to be transferred to a hospital reasonably accessible to the Campgrounds. PARENTS OR GUARDIAN WILL BE NOTIFIED ANYTIME THERE IS A MINOR INJURY.

By clicking "register" I agree to the policies outlined for the Ambassador for Christ Camp. Payments for camp will be accepted in the form of credit/debit, cash & money order ONLY. NO CHECKS. We are sorry if this causes you any inconvenience.

CBWC may use the photograph/video in publications or other media material produced, used or contracted by CBWC including but not limited to: brochures, invitations, books, newspapers, magazines, television, websites, etc.

To ensure the privacy of individuals and children, images will not be identified using full names or personal identifying information without written approval from the photographed subject, parent or legal guardian

A person attending a CBWC event who does not wish to have their image recorded for distribution should make their wishes known to the photographer, and/or the event organizers, and/or contact CBWC at 2660 Rhoads Avenue, Columbus, Ohio 43207 in writing of his/her intentions and include a photograph. CBWC will use the photo for identification purposes and will hold it in confidence.

By participating in a CBWC event or by failing to notify CBWC, in writing, your desire to not have your photograph used by CBWC, you are agreeing to release, defend, hold harmless and indemnify CBWC from any and all claims involving the use of your picture or likeness.

Any person or organization not affiliated with CBWC may not use, copy, alter or modify CBWC photographs, graphics, videography or other, similar reproductions or recordings without the advance written permission of an authorized designee from CBWC.
